Program scope and billing
Choose your program to see what’s included, who pays and when you’re charged.
HCO, HHA and larger employer programs have separate prices and billing terms.
California HCO: a completed TB file.
Your HCO TB service includes:
- Prior TB records requested with the caregiver’s authorization
- Physician review of records and the health questionnaire
- Coordination of one Quest TB blood test, when needed
- A signed TB certificate and supporting result in your dashboard
Standard agency-paid pricing: $69 from records or $99 with a new Quest test.
First-caregiver offer: separate introductory prices for one designated caregiver per new agency. See your offer page.
Caregiver-paid program: separate introductory and time-limited launch terms apply. Standard agency-paid prices do not replace your offer.
Your caregiver-paid offer: first caregiver $0 from records or $69 with a new Quest test; then $45 from records (through March 20, 2027) or $99 with a new Quest test. The caregiver pays only after the physician signs; your agency pays $0.
The prices are alternatives, not added together. Booking alone does not trigger the new-test price. Chest X-rays and other outside follow-up cost extra.
HCO billing depends on who pays.
When your agency pays
We invoice after the signed TB certificate reaches your dashboard.
Positive blood test: the applicable new-test price is billed before clearance because the test was completed. The physician signs after follow-up shows no active TB.
When the caregiver pays
We charge the caregiver only after the physician signs—even after positive-test follow-up. No signed certificate, no charge.
Your agency pays $0 unless it chooses to pay for that caregiver.

Your $99 HHA one-hire offer includes:
- An online health questionnaire
- A Quest TB blood test, when needed
- California physician review and signature
- The health assessment and supporting TB documentation in your dashboard
The worker pays $0.
Your agency is billed after the completed HHA file is delivered. HCO records and new-test prices—and its positive-test billing exception—do not apply to this offer.
Larger programs start with agreed requirements.
For hospitals, FQHCs, health systems and staffing organizations, we agree on these details before starting:
- Required checks and forms
- The workers included
- Pricing
- Renewal services
HCO and HHA one-worker prices do not automatically apply.
What happens after the first file?
Continue with more workers or an agreed group. Tracking renewal dates does not order a test or grant clearance.
Agency-paid HCO renewals: $99 when required, generally on a two-year cycle. Prior positive results need physician review rather than routine repeat screening.
